What we actually did, with figures and how they were counted
Outcomes measured against the stated indicators
Beneficiary feedback, including criticism
What did not work, and what we changed as a result
Financial summary: income, expenditure and how funds were used
Partner contributions and acknowledgements
Plans and commitments for the following year

Why we will publish the failures too

A report that lists only achievements tells a reader nothing, because they cannot tell what was left out. Publishing what did not work is what makes the rest of the document credible — and it is how an organisation actually improves. Stepin commits to this in advance, while it is still easy to commit to.
